Job Title: LVN or RN for In-Home Detox - Per Diem
Location: Travel Assignments and Los Angeles area
Pay: $40–$50 per hour (Based on licensure, experience, and case complexity)
Schedule: Per Diem / As-Needed (12 hr shifts)
Travel: 5–10 day travel assignments available (lodging + travel covered)

Position Overview

We are seeking an experienced Detox LVN/RN to join our concierge in-home detox team on a per-diem basis. This role is best suited for a nurse with strong detox experience—preferably in higher-end, luxury, or high-acuity settings —who is comfortable working 1:1 with clients in a private home environment.

Ideal candidates either:

1. Are open to travel for short-term (5–10 day) assignments throughout California or other designated locations.
2. OR, Live in the Greater Los Angeles area and can take local in-person shifts.

This is a unique opportunity for a seasoned detox nurse who values autonomy, clinical excellence, and individualized patient care.

Key Responsibilities

• Provide safe, compassionate, and comprehensive in-home medical detox support for clients experiencing withdrawal from alcohol, benzodiazepines, opioids, and other substances (based on assignment).
• Perform initial nursing assessments, vitals monitoring, and ongoing withdrawal symptom evaluations (CIWA, COWS, etc.).
• Collaborate with prescribing clinicians to implement medication-assisted withdrawal protocols.
• Maintain thorough, accurate documentation for every shift (progress notes, vitals, symptom updates, incident reporting).
• Monitor for clinical changes and intervene appropriately, including escalation to medical providers when necessary.
• Deliver an elevated level of professionalism and discretion consistent with high-end concierge detox services.
• Travel for multi-day in-home detox assignments as needed (5–10 days at a time), depending on candidate availability.
• Provide emotional support, reassurance, and therapeutic communication throughout the detox process.
• Ensure client safety, clear boundaries, and a stable environment during detox.

Requirements

• Active LVN or RN license (unrestricted)
• Must have detox experience —preferably in a high-end, private-pay, residential, or hospital-based detox environment.
• Strong understanding of withdrawal protocols, medication support, and crisis management.
• Ability to work independently in a home environment with minimal supervision.
• Professional, calm, and discreet bedside manner.
• Must be comfortable with travel assignments (5–10 days) or live locally in Los Angeles for in-person per diem shifts.
• CPR/BLS certification required.
• Clean driving record and reliable transportation (for local shifts).
